Support Coordination

What is Support Coordination and how do I access it?

If your NDIS plan has allocated funding for support coordination, you are eligible to work with one of our friendly support coordinators to receive ongoing support and assistance in your daily endeavours. We have coordinators based in NSW and Queensland who are able to help you as needed. 

 

They help you to build your skills and confidence to a point where you can have greater control over your own NDIS experience. Ultimately, their goal as your support coordinator is to help you access community services, understand your NDIS plan and get the most out of your funded support services. They navigate the local community for you and help you choose the most appropriate providers for your plan – and the choice is always yours.

 

If you want Support Coordination in your NDIS plan and it is not funded, you can ask your LAC or Planner to include it when you come to plan review time.

 

What does your Support Coordinator do?

Your Support coordinator is an industry specialist whose role in your NDIS plan is to help you match with supports that excel in the type of care and outcomes you are seeking. Support coordinators also work with your providers and your plan manager to ensure that your budget is spent correctly, that your providers are helping you work towards your goals, and to see the best outcome possible for you.

 

They also guide the process of having major equipment and/or home modifications costed, defined, trialed, funded and purchased through your plan. They will work closely with your larger care and therapy team and coordinate with them for better outcomes for you.

 

Support Coordinators work to establish assistance within the participant’s home or community to develop skills, such as attending appointments, shopping, bill paying, taking part in social activities and maintaining contact with others. They have active involvement in planning supports wherever there is a need for ongoing maintenance support.

 

Your coordinator will also work with your team if your needs change to pull together the evidence needed to request a change of your plan to the NDIS.

How do I see it in my plan?

Support Coordination is in the capacity building section and is quoted as hours per year. If it is not in that section, then you are not able to access that service.

How often do they contact me?

You and your Coordinator will agree to a schedule of regular phone or in-person catch ups. They will also be in touch before your plan review to coordinate reports and recommendations for the next plan for you.

Mikaela is our dedicated NDIS Support Coordinator with 2 years experience in the role and a strong background in the disability sector and social services. Mikaela is passionate about disability advocacy, helping individuals achieve their goals, building capacity, providing person centred guidance and support, helping people understand their rights and helping people live their life the way that they choose. She is also very passionate about educating people with disabilities and their families around navigating the complex NDIS system

Rachel is our newest support coordinator in the Bathurst region. She has a positive and nurturing approach to supporting her participants to maximise their NDIS plan to achieve their full potential. Her background is support work and she has three children, and has lived experience with disability.

She is a really strong advocate and passionate and through her lived experience with the NDIS and having someone close to her accessing the NDIS due to disability. She is  professional, attentive, caring and understanding.
